An excellent starting point for any caregiver is a call to your local ASAP`s information and referral department to learn about all the services and resources available in your area. The ASAP information and referral department can provide valuable guidance and assistance throughout your caregiving journey. Your town’s Council on Aging is another essential tool to help you understand and access available resources for your loved one.

Aging Services Access Points, commonly referred to as ASAPs, are one-stop entry points for all of the services and benefits available to seniors in Massachusetts. ASAPs manage the Massachusetts Home Care Program, Meals on Wheels, Money Management Program and Caregiver Support Program, as well as provide information and referrals for senior services such as home care, housing options, public benefits, legal assistance, hospice care, the ombudsman program, health insurance, senior companions and adult day care. BayPath also serves as a federal Area Agency on Aging (AAA), receiving and disbursing federal funding under the Older Americans Act.

Councils on Aging (COAs) are local volunteer organizations that offer information and direct services to seniors, their caregivers and other people with aging issues. COAs are part of the local government, and work with other senior agencies and city/town departments to provide social, recreational, health, safety and educational programs for seniors in their communities. Most COAs operate Senior Centers for area residents age 60 or older. COAs are also home to each town’s SHINE Program representatives, who provide free, confidential and unbiased health insurance information, assistance and counseling.

HESSCO Elder Services’ nutrition program operates a Meals on Wheels program serving the south Norfolk County area. Home delivered hot lunch meals are available five days a week to people 60 years of age or older, and their spouses, who have limited mobility and are unable to prepare meals because they lack the physical, emotional or cognitive ability to attend a meal site or prepare a nutritious meal.

The COA provides transportation for shopping and local medical appointments to any Medfield senior. The available vans seat 8, 11 and 14 passengers, respectively, with the 8 and 14 passenger vans being handicap accessible. The COA helps determine which rider requires more assistance and if the van lift will provide safer access to the vehicle.
Transportation for residents over 60
The Town of Medifield offers rides to older residents to go to the grocery store or run other errands. This transportation services is available to any senior requesting a rid Council on Aging events could alter availability. General operating hours are Monday through Friday from 9:00 a.m. to 3:15 p.m. Grocery store trips as scheduled tp go to Shaw’s on Mondays, Market Basket on Tuesdays, and Roche Bros. on Friday, The van is scheduled for errands on Tuesday and Thursday. Costs: Rides to The Center are free to Medifield residents, a $2 donation is suggested for trips to the grocery trips and errands. |
Medical Essential Transportation (MET
The Medical Essential Transportation program (MET), provides door-to-door rides for pre-scheduled medical appointments. These rides are offered through the COA Volunteer Driver Program, but may use other COA vehicles. YOU CAN NEVER SCHEDULE TOO EARLY! There is no guarantee that a ride will be available for last minute schedulers. NO CALL CAN BE LATER THAN 5 BUSINESS DAYS PRIOR TO YOUR APPOINTMENT Please call between Tuesday and Thursday before 3:00 p.m. When calling, please provide your names, address, telephone numbers, appointment time and its location. |
